As part of the North West London Access Improvement Programme, we are implementing a structured plan to enhance patient access to high-quality, timely, and personalised care. The following key areas outline our current focus:
Timely Access to Care
We are increasing appointment availability, including during evenings, weekends, and Bank Holidays, via our Enhanced Access hubs in collaboration with the WL GP Federation. For urgent clinical issues, we aim to provide a same-day or next-day response.
Continuity of Care for Complex Needs
Patients with ongoing or complex health conditions are supported with an emphasis on continuity of care, ensuring access to the same clinical team whenever clinically appropriate.
Optimising Digital Access
We encourage the use of the NHS App and our digital consultation platforms to facilitate efficient triage, remote consultations, and ongoing management of health conditions from home.
Personalised Navigation and Support
Our trained Neighbourhood Navigators are available to direct patients to the most appropriate service – whether that involves a clinician, allied health professional, or community-based resource.
Continuous Quality Improvement
We actively monitor service performance and incorporate patient feedback to inform service development and ensure ongoing improvement in access and patient experience
